Functional medicine is an approach that looks for the root cause of a patient’s symptoms or disease. It asks the question - why? Here at Ami Swanson Functional Medicine, we use a holistic, integrative approach to healing that involves lifestyle modifications as well as the use of advanced diagnostic testing, when appropriate, to get to the root cause.
Ami Swanson NP, LLC does not accept insurance. By partnering with you, not your insurance company, I am able to provide hassle-free and transparent services. This includes telling you up-front exactly what the cost of all services will be, allowing you to invest in your long-term health.
We accept all major credit cards, including HSA and FSA accounts.
We can provide a superbill after your appointment which you can submit to your insurance company for possible reimbursement. The superbill includes Procedure codes and Diagnosis codes.
Your first step is the Foundational Assessment—a comprehensive evaluation designed to understand your health story and lay the groundwork for your personalized care plan.
The Foundational Assessment includes:
A 60-minute initial intake visit with Ami Swanson, Nurse Practitioner
A detailed functional health evaluation
A physical exam
A 45-minute follow-up visit to review labs
An 8-part personalized health plan
At your initial visit, Ami will review your intake forms, listen to your health story, and order lab work based on your needs. She’ll walk you through how and where to complete your lab testing. A couple of weeks later, you’ll return for your follow-up visit to go over your results and receive your individualized plan. This includes recommendations for nutrition, movement, sleep, stress, supplements, and insights into the likely root causes of your symptoms.
This assessment includes two total visits and is a one-time investment of $399.
There’s no obligation to continue after this step. If you decide to move forward, we work within a 6-month membership model to provide the time, support, and accountability needed for meaningful healing.
Functional medicine is rooted in long-term lifestyle change—and real healing takes time, consistency, and support.
Our 6-month membership is designed to give you the time and structure needed to complete your personalized workup, implement your care plan, and assess your progress through follow-up testing. Most patients find this timeframe essential for building momentum and seeing meaningful results.
After your initial membership, you'll have the option to continue working with Ami through ongoing membership options, or follow up visits, tailored to your needs and goals. Many women choose to continue membership for support in deepening their healing, maintaining results, or navigating new concerns as they arise.
Great question! At the end of your initial membership, you have two options depending on your needs and goals:
1. Continue with another 6-month membership
Many patients feel significantly better after six months but want continued guidance to keep optimizing their health or maintain the accountability that helped them succeed. Renewing for another 6 months allows you to continue working closely with Ami in a supportive, trusted patient-provider relationship. This extended time helps solidify lasting lifestyle changes and provides space to go deeper with your healing journey.
2. Transition to individual follow-up visits
If you’re feeling great and ready to move forward on your own, you can switch to as-needed 45-minute follow-up visits. Ami will recommend a follow-up schedule based on your specific situation and health goals.
At a minimum, we suggest an annual Foundational Assessment visit to maintain the progress you’ve made and adjust your plan as needed.
You are required to have a primary care provider. Ami Swanson Functional Medicine offers functional medicine consultation only and does not function as your primary care provider. You must have a primary care provider for yearly physicals, wellness visits and for after-hours or urgent care purposes. We do not respond to urgent requests of any sort, including messages or phone calls, you must have a primary care doctor for this purpose.
No. While lab testing is helpful, it isn’t necessary for everyone. Lab fees are an additional cost and I will discuss costs of any labs prior to ordering them.
We have negotiated reasonable cash prices for labs.
Short answer, it depends! If you’ve had recent labs, you can upload them in your patient portal for me to review prior to our visit. We can then discuss if repeating them would be necessary. Many conventional medical lab panels are not as comprehensive as those I will recommend, and repeating labs is often helpful to see how you've progressed.
If, during our assessment, we uncover that your healing journey involves more complex concerns—such as mold toxicity, Lyme disease, heavy metal detoxification, active cancer, or highly specialized hormone replacement therapy—I will guide you to the right next step. I work closely with a trusted network of integrative and functional medicine providers who specialize in these areas to ensure your care continues seamlessly.
There may also be times when I refer you back to your primary care provider or within the traditional healthcare system for more acute or urgent concerns that fall outside the scope of functional medicine. My goal is always to make sure you’re fully supported with the right resources, at the right time, by the right professionals.
If you're located in Iowa, all appointments can be done virtually if appropriate for your symptoms and preferences. For those outside of Iowa, or for anyone who prefers in-person care, visits take place at our clinic in Spirit Lake, Iowa.
Telehealth is a convenient option, especially for follow-up visits, and many patients find it easy to integrate into their routines.
Once you're enrolled in the membership, you'll also have unlimited direct access to Ami through our secure patient portal for ongoing questions, updates, and support between visits.
